We want to be as prepared as possible for your arrival, so we’ve provided a special phone number for you to call to let us know about your plans. We encourage you to pre-register before your special day. You can complete the form below or call 661.200.1050. Monday through Friday between the hours of 8 a.m. and 6 p.m. or Saturdays or Sundays between the hours of 8 a.m. and 4:30 p.m. Please have your insurance information handy.

Once you are pre-registered and would like to speak with a Financial Counselor, please call 661-200-1056, Monday - Friday between the hours of 8 a.m. and 4:30 p.m., or Saturdays or Sundays between the hours of 7 a.m. to 3:30 p.m.
