You can access your electronic health information by doing the following:

ENROLL TODAY. Please have the following information ready. If you need assistance with any of the items below, please call 661.200.CARE (2273)

• Exact Name spelled as it appears on your medical record
• Date of Birth
• Email address if you provided one when you registered as a patient

Medical Record Number as it appears in your discharge packet or the last 4 digits of your Social Security number

• SIGN IN using the user name and password that you created when you set up your account
