DAISY Award

Want to Say Thank You To Your Nurse? Share Your Story!

Nominate your nurse for the internationally recognized DAISY Award.
The DAISY (Diseases Attacking the Immune System) Award is an international recognition program that honors and celebrates the skillful, compassionate care nurses provide every day. The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease ITP in 1999. During his hospitalization, they deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way. Please say thank you by sharing your story of how a nurse made a difference you will never forget!

To Nominate an Extraordinary Nurse, please complete the form below.

Each DAISY Award Honoree will be recognized at a public ceremony in her/his unit and will receive a beautiful certificate, a DAISY Award pin, and a hand-carved stone sculpture entitled A Healer’s Touch.
Additionally, everyone in the unit will celebrate with cinnamon rolls – a favorite of Patrick’s during his illness. The Barnes Family asks that whenever and wherever nurses smell that wonderful cinnamon aroma, they stop for a moment and think about how special they are.

If you have any questions, please call (661) 200-1029.

Past Nominations

"I wish to stay anonymous in this nomination for personal reasons. The nurse went above and beyond during my family members stay. She was not only an excellent nurse, but a compassionate human. She took care of her patients in a way that most nurses would think to be below their job responsibilities. She took exemplary care of her patient in their darkest moment all while being pregnant!!! That woman is super woman and she deserves every recognition in the world." - Henry Mayo Patient

"My mom suffers from a rare neurological condition that prevents her from being able to communicate properly, among other things. On our first day in the unit, the nurse was the day-shift nurse that day and for the next two days. The nurse did his best to understand my mom and her condition, even taking the time to research it and share his findings with our family. He was so communicative with the ever-changing plan of care and was always in contact with the team of doctors, pushing for better methods of keeping my mom comfortable. His presence and actions made my mom feel cared for and made her visit at Henry Mayo all the better. On our day of discharge, he even stopped in to talk with my mom and us (myself and several family members) and said goodbye to her. The nurse is an outstanding care taker and an exceptional person. He is more than deserving of the Daisy Award." - Henry Mayo Patient

"I had the privilege of personally witnessing a nurse's exceptional care and dedication. She showed incredible warmth, compassion, and expertise, which made a significant difference in the lives of her patients. One particular instance that stands out to me is when I had a Fistulotomy surgery last year, and the nurse was one of the staff nurses. Even after the surgery, she continued to show her compassion and care by being available to talk and provide support. Krista always goes above and beyond, whether it's ensuring the comfort of her patients, offering a listening ear, or providing top-notch medical assistance. Her unwavering commitment to helping others is truly inspiring. I want to express my gratitude to this nurse for her selflessness and for the positive impact she has made on so many lives. She is a true healthcare hero!" - Henry Mayo Patient
